About Us
- The Hospice of the Sacred Heart program was developed for the purpose of providing palliative supportive home care services to patients and their families faced with a terminal illness. The hospice interdisciplinary team consists of a Medical Director, professional nurses, Medical Social Workers, coordinator of volunteers, counselors, chaplain, and volunteers. Additional support for the individual patient may include home health aides, Physical Therapists, Occupational Therapists, Speech Pathologists, and Registered Dietitians. The focus of the interdisciplinary team is the provision of comprehensive home care services to the dying patient and their family. Following the death of the patient, the surviving family is eligible for bereavement counseling support for up to one year.
- The goals of the hospice program are to allow a patient to live out his or her days in an environment of choice, be surrounded by familiar persons, be free of pain, live with mental alertness, and have professional help in coping with the physical and emotional stresses associated with the dying process.
